About the position

The Prevention & Wellness Health Navigator (PWHN) will be responsible for outreach and health navigation services targeting LGBTQIA+ individuals, particularly focusing on Black, Indigenous, and People of Color (BIPOC) communities. This role aims to provide prevention, education, and health access services, with a commitment to reducing the risk of HIV and substance use disorders. The PWHN will work closely with other health navigators and community organizations to ensure comprehensive support for individuals at risk.

Responsibilities

  • Conduct outreach and recruitment activities with target populations in prevention, screening, and treatment.
  • Perform health navigation services for those in need, including supported referrals to address the full spectrum of patient needs.
  • Work with the Senior Director of HIV Services and the Trans Health Navigator to develop strategies for outreach and service delivery.
  • Screen, interview, and counsel individuals who feel they are at risk for HIV/HCV/STI infection.
  • Collect specimens for HIV/HCV/STI tests and transport them to the State Laboratory.
  • Collect data and complete required paperwork for monthly reports.
  • Initiate and conduct follow-up activities to assess community and individual-level impact of provided services.
  • Develop, provide, and document appropriate referrals for clients in need of services.
  • Provide and distribute materials to increase awareness of HIV transmission.
  • Facilitate group level interventions and health communications with approved curriculum.
  • Coordinate and maintain strong working relationships with other community-based organizations.
  • Attend meetings, conferences, and training to expand knowledge of program subject matter.
  • Be available for occasional evening and weekend outreach activities.
  • Establish and maintain good rapport with staff, clients, and other program-related individuals.
  • Perform duties consistent with agency/program policies and procedures.

Requirements

  • Self-identify as or have demonstrated experience with LGBTQIA+ individuals, particularly BIPOC communities.
  • AA/AS desired; High School Diploma or GED plus outreach/education experience strongly desired.
  • Knowledge of HIV/AIDS and Harm Reduction principles and practices; lived experience strongly preferred.
  • Knowledge of the LGBTQIA+ community within the program service area.
  • Demonstrated computer skills and ability to utilize various social media platforms.
  • Strong organizational, time-management, and communication skills (oral and written) required.
  • Demonstrated ability to take initiative and work collaboratively.
  • Approachable personality and open-mindedness required.
  • Resident of or familiarity with the Brockton area strongly preferred.
  • Valid driver's license and own means of transportation.
