Responsibilities
- Assess, plan, and implement individualized care strategies for patients.
- Identify and initiate referrals for social service programs, including financial and psychosocial support.
- Manage the care plan through the continuum of care as a single point of contact.
- Communicate health-related information to stakeholders to ensure quality care.
- Advocate for patients and families to represent their needs and choices.
Requirements
- Current, unrestricted independent licensure as a Registered Nurse in New Jersey.
- 2+ years of clinical experience.
