About the position

The Licensed Practical Nurse (LPN) in the Patient Aligned Care Team (PACT) provides essential support in the Primary Care Outpatient Clinic, focusing on delivering high-quality care to Veterans. The role involves direct patient interaction, data collection, and communication with healthcare providers to ensure smooth clinic operations and effective patient transitions. The LPN is responsible for managing clinic flow, assisting with patient education, and collaborating with team members to develop comprehensive care plans.

Responsibilities

  • Ensure appropriate evaluation and access is provided to patients assigned to the patient panel.
  • Utilize available tools for effective identification and intervention of individual patients and cohorts.
  • Function at the full extent of scope of practice based on experience and training.
  • Manage communications and facilitate safe Veteran transitions between the PACT and other health care settings.
  • Provide health education on wellness, disease prevention, chronic care management, and self-management.
  • Collaborate with informatics technology staff to develop and implement standardized tools for PACT care delivery processes.
  • Participate in PACT team huddles, meetings, and process improvement initiatives.
  • Promote personal, proactive, Veteran-centered care.
  • Assist with pre-visit phone calls to Veterans to complete reminders and obtain reasons for appointments.
  • Review daily clinic schedule for patient care needs.
  • Collaborate with PACT team members to develop comprehensive health care plans for Veterans.
  • Manage clinic flow, including vital signs and treatments as indicated.
  • Complete appropriate clinical reminders and assist providers with exams/procedures.
  • Administer and track immunizations and prepare/administer medications as per licensure.
  • Observe, record, and report patient responses or changes in condition.
  • Recognize and respond to urgent or emergent situations promptly.
  • Assist with orientation of new employees and check daily equipment and supplies.

Requirements

  • U.S. Citizenship is required.
  • Graduate of a school of practical or vocational nursing approved by the appropriate State agency.
  • Full, active, current, and unrestricted licensure as a licensed practical or vocational nurse.
  • Proficiency in written and spoken English is mandatory.
  • Completion of at least 1 year of qualifying experience at the GS-4 level for GS-5 applicants.
  • Completion of at least 1 year of additional qualifying experience at the GS-5 level for GS-6 applicants.

Nice-to-haves

  • Experience in a healthcare setting with Veterans or similar populations.
  • Knowledge of telehealth practices and procedures.
