About the position

The Population Health Outreach Specialist (LPN/CMA) plays a crucial role in an interdisciplinary team focused on enhancing patient care by identifying individuals who are due or overdue for essential health services. This position involves proactive outreach to motivate patients to complete recommended healthcare services, assist in scheduling appointments, and manage disease registries to close gaps in clinical care. The specialist is responsible for conducting chart reviews, supporting quality reporting, and ensuring effective communication with patients and healthcare providers.

Responsibilities

  • Utilizes standardized tools and scripting to conduct patient outreach for scheduling preventative health care services.
  • Conducts pre-visit chart reviews to close gaps in care and ensure accurate risk coding.
  • Manages CMS web interface quality reporting and supports additional quality reporting requirements.
  • Conducts outreach to patients with medication adherence deficits using standard assessments.
  • Identifies and refers high-risk patients to the ambulatory care management team for care coordination services.
  • Practices motivational interviewing to support health behavior change.
  • Requests necessary documents from outside facilities to support care gap closures.
  • Oversees the disease registry, ensuring the database is accurate and up to date.
  • Documents all patient interactions in the health record or care management documentation system promptly after each interaction.
  • Participates in quality improvement activities as directed by the quality manager.

Requirements

  • Iowa state licensure as a Licensed Practical Nurse (LPN) or Certified Medical Assistant (CMA).
  • Minimum of 3 years of healthcare experience, with varied clinical experience preferred.
  • Graduate of an accredited nursing or medical assistant program.
  • Ability to manage confidential and sensitive information with discretion.
  • Strong verbal and written communication skills, with effective interaction capabilities across the care continuum.
  • Demonstrated problem-solving skills and the ability to prioritize in complex situations.
  • Basic proficiency in Excel and Word.
  • Ability to understand and apply guidelines, policies, and procedures.
  • Obtain Certificate of Competency in Population Health Care Management within 12 months of hire.
  • Proof of completion of Mandatory Reporter training within three months of hire.
