Requirements
- Medical license in the state of IL or pending
- PM&R residency
- Board Certified/Board Eligible by the American Board of Physical Medicine and Rehabilitation
- Spine and MSK procedures

Responsibilities
- Work with musculoskeletal, sports and spine patients
- Perform spinal interventional procedures
- Serve patients from Endeavor Service Area (Chicago Metropolitan Market)
- Physician will have at least 35 hours per week of direct patient contact
- Anticipate a volume of 20 outpatients per day once practice has been established
- Physician will be expected to work extended hours
- Flexibility to determine own extended hours (early morning, evenings or weekends)
