About the position

The Physical Therapist will evaluate patients and determine treatment plans, prepare documentation such as evaluations, daily notes, progress notes, and discharge summaries. The role involves consulting with the rehabilitation team to select treatment programs and coordinate services, developing home programs to enhance patient performance, and creating treatment protocols using evidence-based guidelines and patient-specific goals. The therapist will also assist with Quality Assurance reports and facility reports, participate in program development and department meetings, comply with policies, promote company philosophy, ensure quality care delivery, respect patient privacy, confidentiality, and property, and celebrate workforce diversity.

Requirements

  • BS, MS, or DPT degree in Physical Therapy.
  • Current Healthcare Provider CPR certification.
  • Mandatory Reporter of Child and Dependent Adult abuse training certification (within 30 days of hire).
  • Ability to work independently and exercise sound decision-making.
  • Ability to accept and work with diverse populations, culturally, economically, and educationally.
  • Must have a valid driver's license, auto liability insurance, and reliable transportation if driving is required.
