About The Position
Navitas Healthcare, LLC is seeking a Physical Therapist – SNF for an exciting Travel or Local job in Mauston, WI. This is a Full-Time/ Direct Hire position with variable shifts. The role involves evaluating and treating patients experiencing mobility limitations, pain, weakness, or movement dysfunction. The Physical Therapist will develop individualized treatment plans focused on restoring function and independence, assist patients in recovery following surgery, illness, or injury, and help reduce fall risk and improve overall patient safety. The position also supports discharge planning and continuity of care, requires maintaining accurate and timely clinical documentation, and coordination with physicians, nursing staff, insurance providers, and rehabilitation team members. The Physical Therapist will also participate in maintaining a clean and safe treatment environment and perform additional duties as assigned.
Requirements
- Graduate of an accredited Physical Therapy program
- Wisconsin PT license or ability to obtain upon acceptance
- Strong communication and patient-care skills
Nice To Haves
- Minimum 1 year of Physical Therapy experience preferred
- Experience in Skilled Nursing Facility or rehabilitation settings preferred
Responsibilities
