About The Position
Human Touch Home Health is hiring a licensed Physical Therapist (PT) to provide home health services throughout the DMV area, including Washington, D.C., Northern Virginia, and Maryland. This is a flexible PRN opportunity where you can create your own schedule and provide one-on-one care that helps patients regain strength, balance, mobility, and independence.
Requirements
- Active Physical Therapist license in Virginia, Maryland, or Washington, D.C. (multi-state licensure preferred)
- Valid driver’s license and reliable transportation
- Strong documentation and communication skills
Nice To Haves
- Home health experience preferred
Responsibilities
- Complete physical therapy evaluations, reassessments, and discharge planning
- Develop individualized treatment plans and provide skilled therapy services in patients’ homes
- Help patients improve strength, balance, mobility, and functional independence
- Educate patients and caregivers on exercise programs and safety strategies
- Coordinate care with physicians and the interdisciplinary team
- Complete documentation accurately and on time
