About the position

The Physical Therapist (PT) provides high-quality, patient-centered care by managing the complete process of therapeutic care of an assigned group of patients in coordination with the physician and the patient care team. The Physical Therapist completes patient assessments and participates in developing and implementing individualized care plans focusing on goal achievement, patient engagement, and positive patient experience. The Physical Therapist provides or directs a majority of the patient's physical therapy to support patients throughout the continuum of care.

Responsibilities

  • Provide evaluations and treatment interventions within the scope of practice established by law and professional code of ethics.
  • Assess and evaluate therapeutic/rehabilitative/functional status and participate in the development of the total plan of care.
  • Provide purposeful and skilled therapeutic care at patient's home, using procedures and techniques to produce a change in condition/function consistent with the physician's plan of care.
  • Instruct patient/family/caregiver in the use of therapies, exercises, therapeutic appliances, and/or prosthetic devices to be used outside of therapy sessions.
  • Prioritize caseload, determine daily staffing needs and coordinate assignments/handoffs.
  • Analyze medical histories to ensure appropriated treatment.
  • Educate family/patient on treatments precautions and home programs.
  • Assist in the clerical duties of the department and other duties as assigned.
  • Assist in the organizing and supervising clinical affiliation programs for students.
  • Perform administrative duties inherent to the position: supervision of therapy techs.
  • Administer prescribed treatments and evaluation to patients in medical, surgical, and critical care areas.
  • Consult with physicians and other staff regarding the patient.

Requirements

  • Licensed Physical Therapy
  • Bachelors Degree
  • Basic Cardiac Life Support, or CPR for Healthcare Providers, or Heart Saver CPR must be obtained within 30 days of employment start date
