As a Physical Therapist on the Home Care team, you will utilize various exercises to help patients increase strength, range of motion, endurance and coordination within the comfort of their home. Often, your patients are dealing with pain, so sensitivity is important. Because this position requires close interaction, you have the opportunity to build strong relationships with both patients and their families. Ultimately, your unconditional support and reassurance can drive them to work hard and regain their health, allowing you to make a difference in their lives every day.

Responsibilities

  • Perform initial and on-going assessments of patient condition.
  • Establish, revise and evaluate plans of care and involve the patient/family according to their capabilities and desires.
  • Perform therapy interventions.
  • Supervise activities of Physical Therapy Assistants, students and other support personnel.
  • Maintain complete and up-to-date documentation and files at all times regarding patient condition and effectiveness and timeliness of care and interventions.
  • Participate in ongoing interdisciplinary assessment of assigned patients.
  • Participate as a member of the hospice interdisciplinary team when necessary.

Requirements

  • Bachelor's Degree or certificate
  • Graduate of a Physical Therapy Program accredited by the American Physical Therapy Association
  • State Valid Driver's License (VDL) with proof of $100,000/$300,000 coverage upon hire
  • Current state licensure as a Physical Therapist (PT).
  • Current Basic Life Support (BLS) Certification through the American Heart Association (AHA)

Nice-to-haves

  • One year of clinical experience as a licensed Physical Therapist in an acute, rehab or skilled nursing setting
