About the position
The outpatient rehabilitation therapist plays a crucial role in evaluating and providing therapeutic interventions for patients with illnesses or injuries that affect their mobility and daily living activities. This position focuses on developing individualized care plans, setting measurable goals, and ensuring effective communication with patients, families, and healthcare providers to facilitate optimal recovery and independence.
Responsibilities
- Evaluate patients to assess their needs and functional abilities.
- Respond to physician's treatment orders in a timely manner.
- Establish realistic short and long-term goals in collaboration with patients.
- Develop individualized plans of care and communicate them to patients and families.
- Monitor patient progress and adjust care plans as necessary.
- Ensure safety considerations are adhered to during treatment sessions.
- Oversee the clinical performance of physical therapy assistants and aides.
- Document patient evaluations and progress accurately and in compliance with department policies.
- Participate in team and family conferences to coordinate care.
- Facilitate patient orientation to services and expected outcomes.
Requirements
- Licensed as a physical therapist in the relevant state.
- Experience in outpatient rehabilitation settings.
- Strong knowledge of therapeutic techniques and principles.
- Ability to communicate effectively with patients and healthcare teams.
- Proficient in documentation and compliance with regulatory guidelines.
