About the position

The Home Health Therapist, PT plays a crucial role in enabling patients to remain in their homes by providing skilled physical therapy care. This position involves planning and administering treatment for patients with various injuries and functional disabilities, ensuring they achieve the highest level of physical function. The role requires collaboration with physicians and clinical teams to develop and adjust treatment plans, as well as supervising support staff in delivering care.

Responsibilities

• Plan and administer prescribed skilled physical therapy treatment and training for patients.
• Test/screen the patient's physical strengths to assist in evaluating their level of function.
• Develop and implement a conditioning/rehabilitation program consistent with the physician's Plan of Treatment.
• Confer with physician and clinical team members to obtain additional patient information.
• Provide Physical Therapy Assistants and Home Health Aide staff with written instructions/care plans.
• Accurately document patients' care observations, interventions, and evaluations.
• Report patient's progress to the patient's physician, Clinical Manager, staff, patient, and family.

Requirements

• Degree from an accredited Physical Therapy Program (approved by the APTA).
• Minimum of one year physical therapy experience preferred.
• Current and unrestricted Physical Therapy license.
• Current CPR certification.
• Strong organizational and communication skills.
