About the position

The Physical Therapist Assistant at Duke HomeCare & Hospice plays a crucial role in providing physical therapy interventions under the supervision of a physical therapist. This position focuses on delivering compassionate care to patients in their homes or at the inpatient hospice facility, ensuring individualized treatment plans are followed and modified as necessary based on patient progress.

Responsibilities

• Assist the physical therapist in providing physical therapy interventions.
• Demonstrate sound clinical reasoning in treatment planning and monitoring patient progress.
• Modify interventions according to changes in patient status and within the established plan of care.
• Report patient progress to the physical therapist for goal updates and treatment modifications.
• Meet patient care productivity standards consistently.
• Utilize support personnel when appropriate and available.
• Assist in assessing the learning needs of patients and caregivers.
• Ensure patient/family understands treatment processes and follow-up care.
• Document patient care in a timely and efficient manner, ensuring compliance with departmental standards.
• Perform required data collection accurately and within expected time frames.

Requirements

• Associate's degree in physical therapist assistant.
• Current Physical Therapist Assistant license in North Carolina.
• For Level II: Minimum of 36 months of clinical practice experience.
• For Level III: Minimum of five years experience as a Physical Therapist Assistant with two years at DUHS.
