This PRN (as needed) position will float between outpatient pharmacies. The Pharmacist will evaluate medication orders based on patient's weight, allergy history, and compatibilities, and properly record them in the patient profile. They will complete medication orders accurately and prepare them for delivery. The Pharmacist will also clarify and resolve problem orders, contacting physicians or nurses as needed and passing information to the next shift. They will detect problem medication orders, generate alternatives, and effect changes in patient care. Additionally, the Pharmacist will prepare all IV preparations (TPN, central lines, antibiotics, chemotherapy) and respiratory therapy syringes according to departmental guidelines, checking for errors and ensuring timely delivery. They will process chemotherapy orders, ensuring dosages and calculations are checked by two Pharmacists and adhering to all safety precautions for preparing chemotherapy. The Pharmacist will serve as a resource by using current references to answer medication-related questions, reviewing patient data to make recommendations on medication timing, and handling questions and problems professionally. They will anticipate and analyze conflict situations, taking preventative or corrective action, and identifying concerns to appropriate persons to assist in problem-solving. The Pharmacist will function in any area of the central pharmacy where an R.PH. is needed and take administrative initiative to handle problems when a supervisor is not available, serving as an informational resource for the institution.
