About the position

The Permanent Supportive Housing Program Case Manager is responsible for providing support and advocacy for women and children living in a housing program. This role involves crisis intervention, facilitating access to community resources, and maintaining accurate client records. The Case Manager will work closely with clients to help them achieve their goals and ensure compliance with health and safety standards.

Responsibilities

  • Interact with clients and staff members in a manner that is respectful and empowering.
  • Consistent and dependable attendance.
  • Attend all mandatory staff meetings, program meetings, and trainings.
  • Present a professional demeanor to the community through appearance, communication, and presentation.
  • Implement the goals and objectives of the housing program.
  • Provide crisis intervention and support services to women and children living in the housing program.
  • Advocate on behalf of program participants to access community support and resources.
  • Provide Safety Planning.
  • Facilitate Access to community resources and support.
  • Research, prepare, and facilitate groups.
  • Engage in and role model direct conflict resolution and mediation.
  • Address housing issues that require de-escalation and/or mediation.
  • Assist participants in areas such as grocery shopping, food boxes, paying bills, and keeping their unit in a manner compliant with basic health and safety codes.
  • Assist in securing income client qualifies for, such as disability, ATAP, ASAP, etc.
  • Maintain accurate client records and statistics information including the HMIS system as required by the agency and funders.
  • Clean program areas and common areas as needed.
  • Other duties as assigned by the Housing Director.

Requirements

  • Bachelor's Degree in Social Work or closely related field; sufficient job experience may be used in lieu of education.
  • One-year experience with trauma issues, crisis intervention, advocacy, and group facilitation.
  • Valid AK Driver's License.
  • Proficient with computers and ability to do data entry.
  • Ability to lift 40lbs and walk upstairs.
  • Excellent interpersonal, group, and written communication skills.
  • Ability to pass background check.

Nice-to-haves

  • Three years experience in case management.
  • Knowledge of Alaska Culture.
  • Knowledge or prior experience regarding substance abuse and mental health issues.
