About the position
The Hospice House RN is responsible for providing palliative and supportive care to patients and their families, assessing their needs, and coordinating care with the Interdisciplinary Group (IDG). This role emphasizes maximizing patient comfort and enhancing quality of life during critical times, while also ensuring compliance with healthcare regulations and company policies.
Responsibilities
- Assess and identify patient/family needs utilizing the nursing process.
- Coordinate the Plan of Care with the Interdisciplinary Group (IDG).
- Provide palliative and supportive care to the patient/family unit.
- Utilize nursing interventions and teaching skills to maximize patient comfort.
- Educate patients, families, and caregivers about disease processes and palliative interventions.
- Report changes in the patient's condition to appropriate members of the IDG.
- Attend patient deaths and prepare the body for transport.
- Provide bereavement interventions to families and caregivers.
- Supervise patient care given by other nursing team members.
- Participate in Quality Assessment and Performance Improvement activities.
Requirements
- Current RN license in the state providing care or a valid multistate/compact nurse license.
- Minimum one year of in-patient nursing or hospice experience for external candidates.
- Active BLS certification from the American Heart Association or Red Cross.
- Ability to effectively communicate in English (verbal and written).
- Experience with EMR/EHR systems preferred.
- Ability to manage emotional stress of caring for terminally ill patients.
Nice-to-haves
- Experience in teaching or training others.
- Bilingual skills, particularly in English.
