About the position

The Per Diem Home Care Registered Nurse at BrightStar Care of St. Croix Valley is responsible for providing care management services in a home care environment, primarily in the East Metro area. This role involves overseeing client care as directed by the Director of Nursing and Branch Manager, ensuring compliance with quality and safety guidelines. The position requires a compassionate and patient individual who can build trust with clients and their families while performing various nursing duties in the field.

Responsibilities

• Perform new client assessments and reassessments.
• Oversee the care of existing clients and ensure medication reconciliation.
• Provide wound care and coordinate care with skilled service organizations and hospices.
• Make initial evaluation visits and regularly reevaluate clients' nursing and personal assistance needs.
• Assist with the intake of new clients and inform them about services offered.
• Prepare clinical and progress notes and summaries of care.
• Assist in developing organizational goals and performance improvement activities.
• Administer agency policies and procedures and assess staff competency.
• Maintain clinical records in compliance with local, state, and federal laws.
• Promote home health referrals in the healthcare community.

Requirements

• Registered nurse with a current unencumbered license to practice in Minnesota (Wisconsin is a plus).
• Minimum of one year of documented nursing experience required.
• Two or more years of clinical experience preferred, with at least one year in a home health agency, primary care clinic, or health facility.
• Excellent computer skills with experience in digital charting.
• Strong flexibility and time management skills.
• Exceptional nursing assessment skills.
• Ability to comply with professional standards of practice and regulatory compliance such as OSHA and HIPAA.
