About the position

The Patient Services Coordinator-LPN is directly responsible for scheduling visits and communicating with field staff, patients, physicians, etc. to maintain proper care coordination and continuity of care. The role also assists with day-to-day office and staff management. Responsibilities include managing schedules for all patients, editing schedules for agents calling in sick, initiating infection control forms, serving as backup during busy times, maintaining the client hospitalization log, completing requested schedules, processing reassigned and rescheduled visits, and running scheduling reports. The position may also involve receiving lab reports and performing patient visits if clinical.

Responsibilities

  • Manage schedules for all patients.
  • Edit schedule for agents calling in sick, ensuring patients are reassigned timely.
  • Initiate infection control forms as needed.
  • Serve as backup during the lunch hour and other busy times.
  • Maintain the client hospitalization log, including entering coordination notes.
  • Complete requested schedules as tasks appear on the action screen.
  • Schedule discharge visits and OASIS Collection or recert visits.
  • Reschedule declined or missed visits if appropriate.
  • Run all scheduling reports including Agent Summary Report.
  • Prepare weekly Agent Schedules and perform initial review for productivity.
  • Assist with internal transfer of patients between branch offices.
  • Receive lab reports and assess for normality.

Requirements

  • Be a Licensed Professional Nurse or a Licensed Vocational Nurse licensed in the state of practice.
  • Have at least 1 year of home health experience.
  • Prior packet review / QI experience preferred.
  • Coding certification is preferred.
  • Must possess a valid state driver's license and automobile liability insurance.
  • Must possess excellent communication skills and strong organizational skills.
