About the position

The Patient Services Coordinator at Enhabit Home Health & Hospice plays a crucial role in ensuring effective communication and coordination of care for patients receiving home health services. This position is integral to maintaining continuity of care by scheduling patients with field clinicians and liaising with various stakeholders, including patients, physicians, and caregivers. The role emphasizes the importance of a collaborative and innovative work culture, aimed at enhancing patient care in the home environment.

Responsibilities

  • Schedule patients to branch field clinicians.
  • Communicate with field staff, patients, physicians, referral sources, caregivers, and other service providers to maintain proper care coordination and continuity of care.
  • Manage the on-call notebook and hospitalization logs to enhance communication among stakeholders.

Requirements

  • Must possess a high school diploma or equivalent.
  • Must either be a licensed practical or vocational nurse in the state of practice with at least one year of clinical experience in a healthcare setting, or have at least one year of home health, hospice, or pediatric experience within the last 24 months, demonstrating an understanding of staffing and scheduling requirements related to home care services.
  • Must have basic demonstrated technology skills, including operation of a mobile device.
  • Must possess a valid state driver license.
  • Must maintain automobile liability insurance as required by law.
  • Must maintain dependable transportation in good working condition.

Nice-to-haves

  • Previous experience in home health, hospice, or pediatrics is preferred.
