About the position

The Patient Navigator at Sun River Health plays a crucial role in connecting individuals and their families within the target population to essential preventive and primary health care services. This part-time position involves identifying patients in need of various health services, facilitating appointments, and providing support to overcome barriers to care. The role also includes training and mentoring Patient Representatives, ensuring that patients receive the necessary referrals and assistance for their health needs.

Responsibilities

• Identify individuals and families in need of preventive and primary health care services.
• Link patients to necessary health services, including acute, chronic, and communicable disease care.
• Facilitate patient appointments within different departments and outside agencies.
• Consult with medical staff to provide ancillary services such as translation and transportation assistance.
• Assist patients in overcoming obstacles to receiving referrals and care.
• Transmit necessary clinical information to specialists for referrals and obtain consent for information release.
• Review open referrals and provide updates to the clinical team.

Requirements

• High School Diploma or GED required.
• 2-4 years of related work experience.

Nice-to-haves

• Experience in patient service.
• Bilingual in Spanish.
