About the position

The Patient Care Manager at Heartland Hospice is responsible for overseeing the daily operations of the hospice site, ensuring the delivery of quality care to patients and managing clinical and administrative functions. This role involves direct patient care, responding to inquiries, and participating in on-call nursing schedules, all while providing compassionate support to patients and their families.

Responsibilities

  • Oversee day-to-day operations of the assigned hospice site.
  • Administer clinical aspects of the hospice program.
  • Ensure quality of care provision to patients.
  • Manage the site in the absence of the Executive Director or Senior Patient Care Manager.
  • Oversee clinical and administrative recordkeeping functions.
  • Respond to phone referrals and inquiries in a timely and compassionate manner.
  • Participate in on-call nursing schedule and provide direct skilled bedside nursing care in patients' homes.

Requirements

  • Registered Nurse with a minimum of Associate's Degree in Nursing from an accredited school of nursing.
  • Three years' experience in direct patient care in a clinical setting.
  • Minimum one year of management/supervisory experience.
  • Two or more years' experience in hospice or home care.
  • Current license to practice nursing in the state of requested employment.
  • CPR Certification.
  • Current automobile insurance and valid driver's license.

Nice-to-haves

  • Bachelor's Degree in Nursing from an accredited school of nursing.
  • Sensitivity to the feelings of the terminally ill and their families.
  • Ability to communicate effectively with people of all socioeconomic backgrounds.
  • Excellent patient assessment skills.
  • Thorough knowledge of managed care principles and regulatory guidelines.
