Our coordinators play a vital role in connecting patients with care teams and ensuring every patient receives timely, high-quality service. This role is perfect for someone who enjoys multitasking, problem-solving, and making a difference behind the scenes in patient care.
Requirements
- Healthcare experience preferred
- Home Health or Hospice experience is a plus
- Strong communication and customer service skills
- Comfortable using EMR systems and Microsoft Office
- Detail-oriented with excellent organizational skills
- Ability to thrive in a fast-paced environment
Responsibilities
- Assist with incoming Home Health referrals
- Communicate with hospitals, providers, patients, and field staff
- Verify insurance and patient information
- Coordinate staffing and patient care services
- Help maintain accurate patient records and documentation
