About the position

The Patient Care Coordinator (PCC) plays a crucial role in supporting clinical operations within a nephrology and hypertension medical practice. This position involves various clerical duties aimed at ensuring efficient communication and workflow between physicians, patients, and other healthcare staff. The PCC is responsible for greeting patients, managing incoming referrals, and maintaining medical records, all while providing excellent customer service and adhering to confidentiality standards.

Responsibilities

  • Greeting patients professionally both in person and on the phone for front desk duties.
  • Quickly answering or properly referring questions and issues to the appropriate person/team.
  • Processing all incoming faxes: sorting and distributing appropriate information for physicians and nurses within 24 hours.
  • Creating tasks for incoming referrals and medical records for the appropriate team for all provider orders written at the time of appointment.
  • Registering new patients and updating current patient information in the PM system from incoming referrals and faxes.
  • Handling phone coverage and assisting nurses with calls to patients that do not require clinical information.
  • Providing information that enhances the workflow throughout the practice.
  • Establishing and maintaining rapport with hospital and community contacts for medical records.
  • Maintaining incoming and outgoing medical records.
  • Working effectively with patients and staying until the last appointment is completed.
  • Performing additional duties as assigned.

Requirements

  • High school diploma or equivalent required.
  • Two years of experience in a healthcare environment as a Certified Medical Assistant.
  • Knowledge of medical terminology and experience with medical practice software preferred.
  • Proficiency in Microsoft Office Suite and medical software.
  • Strong typing and computer skills.
  • Excellent verbal and written communication skills.
  • Strong attention to detail and multitasking abilities.

Nice-to-haves

  • Experience with EMR systems.
  • RMA certification.
