About the position

The Pathways Care Coordinator is responsible for facilitating the coordination of care for clients and their families who are part of the Pathways to Success Program. This role involves providing holistic support through High-Fidelity Wraparound Services or Intensive Care Coordination Services, ensuring that clients receive the necessary services in accordance with established guidelines. The position requires active engagement with families, community partners, and various stakeholders to promote effective treatment outcomes and advocacy for clients' rights.

Responsibilities

  • Support an active caseload of 10 or 25 families, maintaining weekly contacts through various methods.
  • Document services according to Medicaid rules and the Pathways to Success Program guidelines.
  • Provide services in accordance with the High-Fidelity Wraparound Model and System of Care principles.
  • Participate in mobile crisis response events and coordinate with crisis responders.
  • Network with collaterals for referrals, discharge planning, and advocacy within various systems.
  • Collaborate with community partners on Wraparound plans and treatment goals.
  • Convene and work with child & family teams, promoting family voice and choice.
  • Discuss cases with supervisor in weekly clinical supervision sessions.
  • Complete all required documentation and agency administrative paperwork.
  • Attend clinical meetings and participate in Performance Quality Improvement efforts.
  • Educate the community about the Pathways to Success Program and assist in developing marketing materials.

Requirements

  • Bachelor's degree in Psychology, Social Work, or a related field is required.
  • Ability to work two evenings a week.
  • Experience in community counseling services and working with children and adolescents with mental health needs is desired.
  • Cultural competency and ability to work with diverse backgrounds is essential.
  • Bilingual in Spanish is preferred.

Nice-to-haves

  • Experience with High-Fidelity Wraparound Model and System of Care principles.
  • Familiarity with Medicaid documentation requirements.
