About The Position

The PACT Team Coordinator (Licensed Mental Health Professional or LMHP) in conjunction with the PACT Program Manager leads a multidisciplinary team serving those with severe mental illness. The PACT Team Coordinator duties include providing leadership, assessing, developing, coordinating, and providing treatment and rehabilitation and support services. The role directs the day-to-day clinical operations of the PACT team, including managing a team of clinicians, program budgeting, reviewing documentation, and providing licensed clinical supervision to appropriate staff.

Requirements

  • Master’s Degree from a program that prepares individuals to practice counseling (60-hr Master’s Program), which is offered by a college or university accredited by a regional accrediting agency. (Re: 54.1-3500 of the Code of Virginia)
  • Two years of post-licensure clinical experience and one year of supervisory experience, preferably in a mental health setting.
  • LCSW, LPC or LMFT
  • Knowledge of and adherence to existing professional ethical standards.
  • Knowledge of and adherence to agency policies and procedures.
  • Knowledge of and adherence to licensure regulations related to services provided.
  • Ability to use a personal computer and other office equipment.
  • Willing and available to work modified schedules (weekends, holidays, etc.) as required.
  • Ability to drive and must possess a valid driver’s license.
  • Ability to participate in and successfully pass behavioral intervention training; includes both verbal de-escalation skills and physical skills techniques to be utilized with consumers as needed.

Responsibilities

  • Providing leadership, assessing, developing, coordinating, and providing treatment and rehabilitation and support services.
  • Directing the day-to-day clinical operations of the PACT team.

  • Monitoring client movement in the therapy process.
  • Terminating the therapeutic relationship in appropriate ways.

  • Effectively communicating orally, both in person and by telephone.
  • Effectively communicating in writing and preparing written documents.
  • Maintaining a consistent and dependable work schedule.
  • Traveling to meetings, other campuses, and counseling sites.
  • Responding to emergency situations in a timely and appropriate manner.
  • Using a personal computer and other office equipment.
  • Providing clinical supervision to QMHP-Es, Interns, and/or LMHP-Es.
  • Working modified schedules (weekends, holidays, etc.) as required.
  • Driving and possessing a valid driver’s license.
  • Traveling to any Horizon worksite.
  • Transporting clients.
  • Providing services to clients at other locations.
  • Attending trainings and Horizon meetings which may require travel to a variety of Horizon sites.
  • Participating in and successfully passing behavioral intervention training, including verbal de-escalation skills and physical skills techniques.
