About the position

Moments Hospice of Chicago South is seeking compassionate Registered Nurses (RNs) for an On-Call position. This role involves providing skilled nursing care to patients and their families in a hospice setting, focusing on palliative care and symptom management. The position operates on a 7 days on, 7 days off rotation, allowing for a balanced work schedule while ensuring that patient care needs are met promptly.

Responsibilities

• Provide skilled professional nursing care to patients, families, and caregivers.
• Oversee and provide primary care, including clinical care, palliative care, and symptom control.
• Regularly assess patients for nursing needs and implement appropriate care plans.
• Initiate interventions and support for patients and families upon admission.
• Re-evaluate nursing needs to ensure comprehensive care plan interventions.
• Collaborate with the interdisciplinary team to meet all patient and family care needs.
• Work with the Patient Care Manager in planning, implementing, and evaluating medical and nursing care.

Requirements

• Current and unrestricted Registered Nurse licensure.
• Bachelor's degree in nursing.
• C.H.P.N. (Certified Hospice and Palliative Nurse) certification preferred.
• Minimum of 1 year of general nursing experience, preferably in hospice, home health, medical, surgical, or critical care settings.
• Experience with pain and symptom management interventions.
• Ability to work within an interdisciplinary team.
• Understanding of hospice philosophy and principles of death and dying.
