Nurse Transition Care Liaison, Home Health

About the position

As a Transition Care Liaison at AccentCare, you will work one-on-one with clients to provide customized care, developing plans under the guidance of physicians. This role allows you to utilize your nursing skills and techniques to ensure quality care for patients. Responsibilities

• Manage communication channels between physicians, social workers, discharge planners, hospital case managers, Patient Care Navigators, and agency staff.
• Clinically assess, coordinate, and communicate care needed prior to home care admission or resumption of care.
• Conduct bedside visits with patients to facilitate successful transitions to home.
• Provide input and clinical expertise into patient transition and care plan development.
• Build and maintain relationships with hospitalized agency patients to ensure optimal experiences.
• Transition patients to Patient Care Navigators for follow-up care post-discharge.
• Procure physician signatures on written orders regarding patient care and communicate to agency staff.
• Collect and provide relevant information for patient care plans, including demographics and clinical data.
• Assist in timely processing of physician orders.
• Manage and grow referral sources by identifying new ones and educating them on agency services.

Requirements

• Bachelor's degree and 3 years of experience; or equivalent combination of education and experience.
• 3 - 7 years of experience in facility/physician relationships with a deep understanding of facility discharge processes.
• Licensed RN, LVN or PT in practicing state.
• Current driver's license and liability insurance.
