About the position

The Nurse Practitioner at Ultra Health Providers will play a vital role in managing the medical needs of residents in skilled nursing facilities. This position involves working collaboratively with medical staff to provide primary, urgent, and preventive care, while also establishing strong relationships with facility administrators and staff. The role emphasizes independent patient care and treatment of complex medical conditions, contributing to improved patient outcomes and reduced hospital readmission rates.

Responsibilities

• Provide primary, urgent, and preventive care for residents in assigned skilled nursing facility.
• Verify admission orders and conduct initial assessments of residents.
• Conduct physical exams, medication reviews, and depression screenings.
• Collaborate with facility medical director, attending physicians, and nursing staff to deliver high-quality patient care.
• Communicate with and counsel family members on treatment options and material changes in patient status.

Requirements

• Valid Nurse Practitioner license in the state of Kentucky.
• DEA License.
• National certification as a Nurse Practitioner.
• Wound certification.
• Clinical background in adult, family, or geriatric specialties.
• Vaccination against COVID-19 is required for all new hires unless exempt.

Nice-to-haves

• Experience in internal medicine, gerontology, or cardiology.
• Excellent administrative, organizational, and verbal skills.
• Ability to work independently.
• Detail-oriented, dependable, and reliable.
• Nursing experience in ICU, ER, or skilled nursing facilities (SNF) is a plus.
