About the position

Pine Park Health is seeking a full-time Nurse Practitioner to deliver primary care services to older adults in senior living communities across Phoenix, Arizona. This role focuses on providing person-centered care, developing care plans, and collaborating with a multidisciplinary team to enhance patient outcomes. The Nurse Practitioner will work with a smaller patient panel, emphasizing a value-based care model and supporting the overall mission of improving healthcare for seniors.

Responsibilities

• Provide onsite primary care services to older adults living in Senior Living Communities
• Develop and execute detailed person-centered care plans for all primary care patients
• Work with the Medical Director to identify and develop policies addressing day-to-day practice issues
• Provide monthly Chronic Care Management (CCM) visits and review CCM documentation
• Run regular huddles and provide leadership to the clinical team
• Identify ways to collaborate with senior living management and care teams to improve patient outcomes

Requirements

• Nurse Practitioner with geriatrics experience preferred
• Comfortable in a fast-paced and changing environment
• Willingness to provide onsite clinical care across Phoenix, AZ
• Desire to work with complex care patients
• Strong orientation towards collaboration with a multidisciplinary team
• Strong written and verbal communication skills
• Ability to work with tech-enabled care delivery tools
