About the position

The Nurse Practitioner at Elev8 Center will provide comprehensive evaluations and treatment for adult patients dealing with substance abuse and mental health issues. This role involves assessing patients for admission to detox and rehabilitation, determining appropriate levels of care, and addressing ongoing health issues. The Nurse Practitioner will work collaboratively with physicians to develop and implement treatment plans, ensuring patient advocacy and education throughout the process.

Responsibilities

  • Conduct evaluations with relevant history and health assessments based on the patient's signs and symptoms.
  • Identify patient needs and make differential diagnoses based on clinical presentations and findings.
  • Conduct waived testing for urine, blood work, and glucose monitoring.
  • Develop a prioritized and mutually acceptable plan of care in collaboration with the physician.
  • Implement treatment plans with appropriate diagnostic tests, pharmacological and non-pharmacological interventions, and patient education.
  • Evaluate the effectiveness of treatments and modify management plans as necessary to achieve optimal mental health goals.
  • Provide educational materials and information to help patients make informed health choices.
  • Promote patient advocacy at individual, community, and global levels.
  • Maintain accurate, legible, and confidential medical records.

Requirements

  • Master's Degree in Nursing
  • Current/valid New York State Nurse Practitioner (NP) license
  • Currently registered as an NP certification in good standing
  • Certified as a nurse practitioner with prescription writing privileges
  • DEA license
  • Five (5) years of related work experience
  • Current basic life support for health providers (BLS) certification
  • ANCC credentialing and certification
