Responsibilities

  • Utilizes advanced assessment skills to manage a cohort of patients.
  • Performs and documents daily assessments.
  • Collects and analyzes data and formulates a family-centered plan of care.
  • Performs any needed advanced procedures as per the Scope of Practice Document.
  • Evaluates and monitors the effectiveness of the prescribed treatment plan.
  • Modifies the plan of care based on treatment effectiveness and patient/family response.
  • Serves as an educational resource to patients, families, nursing staff, and provider teams.
  • Acts as a consultant to multidisciplinary health care team members.
  • Participates in scholarly activities including research and education.
  • Maintains required competencies and seeks appropriate educational offerings for professional development.
  • Acts as a preceptor and mentor to support the development of nurses, nurse practitioners, students, and other health care providers.
  • Demonstrates a commitment to patient safety by addressing safety issues and enhancing practice. Shares errors or potential errors through system reporting mechanisms.

Requirements

  • Master's Degree in Nursing required.
  • At least two (2) years of experience as a Nurse Practitioner required.
  • Registered Nurse (Pennsylvania) or Registered Nurse (New Jersey) upon hire required.
  • Certified Registered Nurse Practitioner (CRNP) (Pennsylvania) or Advanced Practice Nurse (APN) (New Jersey) upon hire required.
  • Prescriptive Authority (Pennsylvania or New Jersey) within 3 months required.
  • Specialty Certification as a Nurse Practitioner in area of education/subspecialty upon hire required.
