About the position

The Nurse Practitioner position at Matrix Medical Network involves conducting home risk assessments for at-risk health plan members. This role focuses on providing comprehensive health assessments that include reviewing medical history, medication adherence, and social environments to improve overall health outcomes. The position offers flexible scheduling and the opportunity to make a significant impact on the health and well-being of community members.

Responsibilities

• Conduct Adult/Geriatric Assessments, including medical history, diagnosis, treatment, health education, and referrals.
• Work collaboratively with physicians, case managers, social workers, family members, and other medical personnel.
• Collaborate with Primary Care Physicians on patient education and follow-up care.
• Provide services in various venues, including home visits, skilled facility visits, and telehealth visits.

Requirements

• Master's Degree required or commensurate experience and satisfactory completion of NP licensure.
• Current RN and NP licensure in the state of practice, including prescription authority or the ability to obtain it.
• Board certified by the AANP, ANCC, or the AACCN in a Matrix approved specialty.
• Current BLS, ACLS, or CPR certification.
• Minimum of 1 year experience as a Nurse Practitioner.
• Strong computer skills and familiarity with employee health/medical record software.
• Excellent verbal and written communication skills with patients, clients, and colleagues.
• Comfortable and flexible with frequent change.
