About the position

The Nurse Practitioner or Physician Assistant role at ConcertoCare focuses on delivering compassionate, individualized, and holistic patient care to seniors and adults with complex care needs. This position allows for flexible clinical care delivery through both in-person visits and virtual consultations, supported by a multidisciplinary team. The role emphasizes collaboration, patient engagement, and adherence to best practices in a tech-enabled care environment.

Responsibilities

  • Provision of compassionate, individualized, and holistic patient care including comprehensive care, urgent care, and chronic condition management.
  • Collaboration with the multidisciplinary team to deliver transitions of care support and address social determinants of health.
  • Delivery of exceptional clinical care both in-person and virtually in various settings.
  • Integration of clinical practices and operational processes to achieve company performance goals.
  • Promotion of patient engagement and experience initiatives.
  • Maintenance of comprehensive medical records and timely clinical documentation.
  • Active participation in care team huddles and organizational meetings.
  • Collaborative engagement with internal and external care team members for care coordination.
  • Adherence to medical policies, protocols, and clinical guidelines.
  • Supportive function as a growth/engagement ambassador with patients and clinical partners.
  • Participation in after-hours on-call schedule and other clinical care cross-coverage.

Requirements

  • Nurse Practitioner or Physician Assistant with graduation from an accredited health professions school and active national board certification.
  • Active licensure in Kentucky.
  • Active DEA licensure and state-specific controlled substance registration.
  • Certification in basic life support (BLS) required.
  • Clinical experience caring for aging adults and adults with complex care needs required.
  • Ability and means to travel as needed in a timely manner.

Nice-to-haves

  • Advanced training in geriatrics and/or palliative medicine preferred.
  • Clinical experience caring for patients in the home setting preferred.
  • Previous experience with managed care and/or value-based healthcare delivery preferred.
  • Average to advanced computer and software skills required.
