About The Position

Signify Health, part of CVS Health®, is seeking a Per Diem Clinician (Nurse Practitioner or Physician Assistant) to provide In-Home Health Evaluations. This role involves engaging with individuals in their homes to bridge gaps in care and build trusted relationships. The evaluations, lasting about 45 minutes, include a medication and medical history review, a physical evaluation, and potentially additional diagnostic tests if ordered by the health plan. Clinicians use an iPad with an intuitive workflow and are not required to prescribe medicine, order lab tests, or alter current treatment regimens. The role requires flexibility to travel locally and within licensed states, and may involve conducting virtual visits. Services include Diagnostic & Preventive Services products such as spirometry, DEE, and bone density screening. The primary workplace is the private residence of health plan members, which can vary in conditions and may contain household pets, secondhand smoke, and other airborne allergens or irritants.

Requirements

• Active, unrestricted license(s) in coverage area(s)
• Board certification (required)
