About the position

The Nurse Practitioner (NP) role involves delivering preventive care to members in their homes, conducting comprehensive assessments, and creating personalized care plans in collaboration with primary care physicians. This position is part of a large healthcare organization in Mercer County, NJ, and requires a commitment to providing high-quality care to at-risk members transitioning from facilities to home.

Responsibilities

• Deliver preventive care to members in the comfort of their home.
• Complete a comprehensive review and assessment of a member's medical history, physical health, environment, and psycho-social status.
• Document progress notes, prepare member workups, and encounter summaries.
• Help at-risk members transition from the hospital and other facilities to their own home.
• Create personalized care plans in coordination with the member's primary care physician.

Requirements

• Must be based in the state of New Jersey.
• Certified as an Advanced Nurse Practitioner in specialty areas.
• Hold current unencumbered NJ Licensure as a Registered Nurse.
• Active DEA registration.
• Minimum of two (2) years' experience as an Advanced Registered Nurse Practitioner.
• Master's degree in advanced clinical practice nursing or graduate of post Master's certificate program for Nurse Practitioner.
• Current provider card in Basic Life Support (BLS).
• Willingness to travel to the member's home and therefore must have reliable transportation.
