About the position

Optum is seeking a Housecalls Nurse Practitioner (NP) or Physician Assistant (PA) to join the HouseCalls team in Berkeley County, WV. This role involves providing annual clinical assessments to patients in their homes, focusing on preventive care to identify and reduce health risks. The position is part of a broader Home and Community Care team that aims to deliver high-quality medical services and compassionate care to vulnerable populations, ensuring a seamless health journey for patients.

Responsibilities

  • Conduct in-home assessments including evaluation of past medical history, review of systems, medication reconciliation, vital signs, and comprehensive physical exam.
  • Perform evidence-based practice screenings including point of care testing as appropriate.
  • Identify diagnoses for care management and active medical management.
  • Formulate a list of current and past medical conditions using clinical knowledge and judgment based on assessment findings.
  • Communicate assessment findings to inform the member's PCP of potential gaps in care.
  • Identify urgent and emergent situations and intervene appropriately.
  • Educate members on topics such as disease process, medication, and compliance.

Requirements

  • Active NP license or ability to obtain by start date, unencumbered and free of any disciplinary actions.
  • For NPs: Approval to practice autonomously if applicable; for PAs: Ability to work under a Collaborative Agreement in applicable states.
  • Active ANCC or AANP national certification in relevant fields or ability to obtain by start date.
  • Ability to complete physical requirements of the job including lifting a 30-pound bag, navigating stairs, and performing physical assessments.
  • Reliable transportation for home visit assessments.

Nice-to-haves

  • 1+ years of clinical experience in highest level of education or clinical setting.
  • Experience in gerontology, cardiology, internal medicine, or endocrinology.
  • Home health care or home visit experience.
  • Fluency in Spanish, Cantonese, Korean, Vietnamese, Polish, or other languages.
