About the position

Optum is seeking a Nurse Practitioner to join the Home-based Medical Care team in Benton County, WA. This role focuses on providing home-based medical care to complex, chronic patients, particularly older adults, to enhance their quality of life and ensure they can remain at home. The Nurse Practitioner will work collaboratively with integrated medical teams to deliver high-quality, compassionate care that addresses the physical, mental, and social needs of patients.

Responsibilities

  • Establish and maintain effective relationships with patients, caregivers, and other healthcare providers.
  • Perform initial comprehensive assessments on newly enrolled patients and provide ongoing care.
  • Develop and update patient management care plans based on changes in condition or after hospitalization.
  • Consult with Physician Leaders and coordinate with Primary Care Providers (PCPs) as needed.
  • Perform basic episodic nursing care procedures such as IV placement, blood draws, injections, and wound care.
  • Conduct post-discharge visits to ensure patient stability aligns with their care goals.
  • Participate in HEDIS measure campaigns and quality initiatives to improve care management and delivery.
  • Participate in clinical after-hours on-call program on a rotational basis.

Requirements

  • Certification through the American Academy of Nurse Practitioners (AANP), American Nurses Credentialing Center (ANCC), or the National Commission on Certification of Physician Assistants (NCCPA).
  • Current, unrestricted Nurse Practitioner (NP) or Physician Assistant (PA) licensure in Washington state.
  • Active, unrestricted Registered Nurse (RN) licensure in Washington state.
  • Access to reliable transportation and compliance with Optum Motor Vehicle Safety policy.

Nice-to-haves

  • 2+ years of clinical experience in primary care, home-based medical care, geriatrics, palliative care, or emergency/urgent care.
  • Knowledge of long-term care and community resources for patient care delivery.
  • Familiarity with CPT, ICD-10, and HCPC codes.
  • Understanding of clinical standards of care and NCQA requirements.
  • Awareness of CMS guidelines and Medicaid/Medicare contracts.
