About the position
The Bi-lingual Nurse Practitioner at GuideStar Eldercare will play a crucial role in providing care to an aging population suffering from dementia, Alzheimer's disease, and other neurocognitive disorders. This position offers the autonomy of private practice while providing support through an assigned caseload and collaboration with corporate clinical leadership. The Nurse Practitioner will work within a multidisciplinary team to promote patient safety, functionality, and dignity, making significant contributions to behavior health-centered patient care.
Responsibilities
- Travel daily to assigned facilities from home.
- Provide initial evaluations, continuing therapy services, and cognitive testing.
- Document the need and extent of services for each patient.
- Create and maintain positive professional relationships with client facilities, patients, and their families.
Requirements
- Proof of State licensure
- Pass pre-employment background check and drug screening
- Complete credentialing process
- Bilingual English/Spanish required
