As a Nurse Practitioner in Advanced Illness Management (AIM), you will play a crucial role in facilitating advanced illness management and palliative care for Empatia patients. Your responsibilities will include developing comprehensive, individualized treatment plans while demonstrating excellent clinical skills and ethical judgment. You will provide diagnosis, treatment, consultation, and follow-up care under the guidance of the Palliative Medical Director or Primary Care physician. This position requires collaboration with various healthcare professionals to ensure that patients receive the highest quality of care throughout their treatment journey. In this role, you will take responsibility for advanced illness management from the initial assessment through to discharge. Your assessments will cover disease-specific changes, functional decline, nutrition and cognition, pain management, and the overall impact of the disease on the patient's quality of life. You will establish individualized care plans in consultation with patients, families, and the Advanced Illness team, ensuring that all needs are met in accordance with the Plan of Care. Additionally, you will participate in on-call services as required, prescribe medications, order and interpret diagnostic tests, and maintain effective communication with the healthcare team to ensure optimal patient outcomes. Your documentation will be thorough and consistent with medical standards to support sound medical practice and reimbursement for services provided.

Responsibilities

  • Facilitate advanced illness management and palliative care for Empatia patients.
  • Develop comprehensive, individualized treatment plans under the direction of the Palliative Medical Director or Primary Care physician.
  • Conduct initial assessments covering disease-specific changes, functional decline, and other relevant factors.
  • Establish individualized care plans in consultation with patients, families, and the Advanced Illness team.
  • Participate in evening/weekend call as required, providing on-call services competently.
  • Prescribe medications, including controlled substances, in accordance with state law.
  • Order and interpret laboratory, imaging, and other diagnostic tests within the scope of practice.
  • Communicate clinical findings to the PSC Physician as needed, at least bi-weekly.
  • Re-assess the effectiveness of care plans regularly and modify as necessary.
  • Maintain effective working relationships with the supportive care team and other healthcare professionals.
  • Document clinical findings, treatment plans, and care provided in the patient's medical record.

Requirements

  • Minimum of a Master's of Science in Nursing from an accredited school of nursing or equivalent as allowed under state law.
  • Minimum of two years of Nurse Practitioner experience.
  • Nurse Practitioner specialty in Family or Adult Medicine preferred.
  • Value Based Care practice experience preferred.
  • Practice experience with Part B billing.
  • Ability to work autonomously in an Advanced Practice Role.
  • Current license as a registered nurse or equivalent as required by state law for Nurse Practitioner licensure.
  • Current Nurse Practitioner certification/license.
  • Collaborative agreement with a supervising physician as required by state law.
  • Part B Provider certification.
  • Federal DEA license and State DEA license (if required).
  • A current automobile insurance and valid driver's license.

Nice-to-haves

  • Experience in palliative medicine principles and when a patient may be appropriate for home health or hospice care.
  • Knowledge of Medicare/Medicaid regulations and interdisciplinary processes.
