About the position

The Nurse Navigator for Surgery plays a crucial role in coordinating patient care for those undergoing thoracic surgery and lung cancer screening. This position involves acting as a liaison between patients and healthcare providers, ensuring that patients receive timely follow-up care, education, and support services. The Nurse Navigator will facilitate communication among multidisciplinary teams and assist patients in navigating their treatment plans effectively.

Responsibilities

  • Coordinate referrals from healthcare facilities and individual providers.
  • Provide patient education related to illness and treatment management.
  • Act as a community liaison for support programming.
  • Contact patients within two days of discharge for care coordination.
  • Ensure all summary of care documentation is available for pending visits.
  • Provide clinical support, counseling, and education to patients.
  • Work with a multidisciplinary team to develop care plans.
  • Prepare clinic notes for new and returning patients.
  • Pull and scan patient records from Cerner and EPIC into Allscripts.
  • Order lab work and schedule necessary testing and biopsies.
  • Assist providers in addressing care provision insufficiencies.
  • Serve as the point person for the Lung Cancer Screening Program.
  • Perform smoking cessation counseling during screenings.
  • Maintain up-to-date records for the Lung Cancer Screening Program.
  • Reach out to referring physicians to ensure proper communication of results.
  • Follow up with patients regarding scan results and recommendations.

Requirements

  • Bachelor's degree in Nursing.
  • Current D.C. Licensure as a registered nurse or application for licensure in D.C.
  • Minimum of 2 years of experience as a registered nurse.
  • Experience in public health or health/patient education preferred.

Nice-to-haves

  • Experience in oncology nursing.
  • Familiarity with electronic health record systems.
