About the position

The Nurse Navigator (RN) - Breast Health/Oncology is responsible for identifying and managing high-risk patient populations, coordinating their care, and providing safe, age-appropriate patient care within a collaborative team model. This role is essential in ensuring that patients receive comprehensive support throughout their oncology treatment journey.

Responsibilities

  • Identify and manage high-risk patient populations.
  • Coordinate patient care across various services and providers.
  • Provide safe and age-appropriate patient care.
  • Collaborate with a multidisciplinary team to ensure comprehensive care.
  • Educate patients and their families about treatment options and care plans.

Requirements

  • Bachelor of Science in Nursing (BSN) degree is mandatory.
  • 4-5 years of oncology nursing experience required.
  • Experience in an outpatient setting preferred.
  • Maryland Registered Nurse License is required.
  • American Heart Association CPR Certification is required.
  • Oncology Nurse Certification is required.

Nice-to-haves

  • BLS Certification (Preferred)
  • 1 year of nursing experience (Preferred)
