About the position
The Nurse Navigator plays a crucial role in managing the complex care processes for oncology patients from diagnosis through treatment and recovery. This part-time position serves as the primary contact for patients, ensuring seamless navigation through the healthcare system and reducing gaps in care. The Nurse Navigator collaborates with a multidisciplinary team to provide comprehensive support and education to patients and their families.
Responsibilities
- Manage complex care processes for oncology patients from diagnosis to recovery.
- Act as the primary contact for new cancer patients and their families.
- Gather and assess patients' medical, psychosocial, functional, and financial information.
- Communicate with the multidisciplinary team to complete patient assessments.
- Schedule tests, procedures, appointments, and treatments for patients.
- Develop understandable treatment plans for patients and caregivers.
- Educate patients and families on disease, treatments, and side effects.
- Assist patients in overcoming barriers to care, such as transportation and financial issues.
- Coordinate discharge planning with various healthcare providers.
- Link patients to community resources and supportive services.
- Facilitate multidisciplinary care conferences and ensure timely communication on patient care.
Requirements
- Associate's Degree in Nursing from an accredited institution.
- California Registered Nurse (RN) license upon hire.
- Oncology Certified Nurse (OCN) certification within one year of hire.
- Basic Life Support (BLS) certification upon hire.
- 2-3 years of relevant nursing experience, preferably in oncology.
Nice-to-haves
- Three years of oncology nursing experience.
- Experience as an oncology nurse navigator.
- Bachelor's degree (BA, BS, BSN) from a four-year college or university.
