About the position

The Nurse Navigator - Inpatient/Outpatient Transplant and Cellular Therapy at Nemours Children's Health is responsible for providing high-quality service and care coordination for patients seeking outpatient services related to cancer and blood disorders. This role serves as an advocate for complex patients and their families, ensuring they navigate the healthcare system effectively and receive the necessary support throughout their treatment journey.

Responsibilities

  • Coordinate high-level service and quality of care for patients requesting outpatient services.
  • Serve as an advocate for complex patients, families, and caregivers to overcome healthcare system barriers.
  • Improve patient access to care through efficient coordination of admissions, visits, and complex scheduling.
  • Triage newly diagnosed patients and families with complex needs in both inpatient and outpatient settings.
  • Provide procedure-related information during the scheduling process.
  • Collaborate with physicians and healthcare team members to maintain a quality healthcare plan.
  • Guide and educate complex patients and families to ensure they receive necessary care throughout the continuum.
  • Coordinate prior authorizations and peer-to-peer communications for complex patients.
  • Collaborate with specialty nurses for care coordination and cross coverage as needed.
  • Evaluate patient and family responses to care on an ongoing basis.

Requirements

  • BSN from an accredited program.
  • Current, unrestricted DE Registered Nurse License required.
  • Minimum 5 years of pediatric hematology/oncology nursing experience required.
  • Specialty Certification: CPON/CPHON certification preferred.
