About the position

The Clinical Coordinator at DaVita is responsible for delivering high-quality care to patients in an outpatient dialysis center, focusing on those with end-stage renal failure or chronic kidney disease. This role involves coordinating patient care, ensuring safety and comfort, and building meaningful relationships with patients and their families. The position requires flexibility in scheduling and the ability to work in a fast-paced environment, often dealing with patients who have multiple co-morbidities.

Responsibilities

  • Coordinate ESRD patient care and services, including tracking lab values and vital signs.
  • Ensure the clinical team delivers the highest level of care to patients.
  • Build meaningful relationships with patients and their families.
  • Oversee a group of patients with the support of Patient Care Technicians (PCTs).
  • Conduct patient observations and measure statistics.
  • Set up dialysis machines and ensure patient safety and comfort.

Requirements

  • Current Registered Nurse (RN) license in the state of practice.
  • Current CPR certification required.
  • At least 18 months of registered nursing experience.
  • Dialysis Charge RN readiness approval by Clinical Nurse Manager or RN-licensed FA/GFA required.
  • Associates Degree in Nursing (A.D.N) from an accredited school of nursing required; Bachelor of Science in Nursing (B.S.N) preferred.
  • Basic computer skills and proficiency in MS Word and Outlook required.

Nice-to-haves

  • Certified Nephrology Nurse (CNN) or Certified Dialysis Nurse (CDN) preferred.
  • Intensive Care Unit (ICU), Critical Care Unit (CCU), Emergency Room (ER), or Medical Surgical (Med Surg) experience is preferred.
  • Supervisory experience preferred.
