As part of a person-centered care model, and under the direction of the RN, this role will provide skilled nursing care to clients in home and community-based settings ensuring the delivery of outcome based, high quality, cost-effective care. This role will collaborate with all team members to achieve exceptional outcomes.
Requirements
- High School diploma or GED
- Successful completion of a CT state approved LPN certificate program
- Computer skills including but not limited to Outlook, Microsoft Word, and Excel
- Current and valid LPN license
- Valid CT driver’s license & a good driving history
Responsibilities
- Provide intermittent skilled nursing services in accordance with the established plan of care under the direction of an RN.
- Provides support to patient/family utilizing teaching principles to promote and restore health and prevent disease and disability.
- Records daily progress in EMR.
- Reports to primary nurse, supervisor, physician, social worker, or therapist regarding cases.
- Communicates/conferences with primary nurse and other disciplines.
- Consults with physician regarding client needs and plan of care.
- Assists in evaluation of care given by paraprofessional staff.
- Participates on agency committees and special projects.
