About The Position

Union West Hospitalist group is part of Atrium Health’s Carolinas Hospitalist Group, providing inpatient medical care at Atrium Health Union West in Matthews, North Carolina. This team is responsible for managing the care of hospitalized adult patients, working within a patient-centered, team-based care model aligned with Atrium Health’s mission and clinical standards.

Requirements

  • Minimum 2 years of RN experience in an acute care hospital setting, ED or ICU
  • Experience with discharge planning, case management support, or transitions of care
  • Familiarity with EPIC
  • Associate Degree in Nursing with active RN licensure in NC
  • Active and Current BLS certification

Responsibilities

  • Works with provider(s) to create focused patient-centric discharge plans, addressing barriers to discharge for patients with length of stay (LOS) that is longer than expected
  • Coordinates the evaluation, testing, management and follow-up of patients in collaboration with the attending physician, and other members of the healthcare team
  • Rounds with physicians and serves as a liaison between physician, healthcare team, patient and family members
  • Promote and assist with patient and family education supporting the treatment plan and educational needs of the patient/family members to achieve continuity of care as care transitions to the home environment or next level of care
  • Ensure patients have scheduled follow up appointments prior to discharge
