About the position

The Nurse Case Manager (RN) position at Eliot Community Human Services involves providing comprehensive nursing care and case management for adults with severe and persistent mental illnesses. The role focuses on outreach, medication management, and support within clients' homes, ensuring they receive the necessary rehabilitative services to enhance their functioning in various environments. The position requires a commitment to promoting consumer rights and responsibilities while working collaboratively within a multidisciplinary team.

Responsibilities

  • Act as a primary case manager to 3-6 consumers
  • Provide nursing interventions to individuals
  • Assist in the management of medications and the overall functioning of the nursing department
  • Formulate and regularly update Comprehensive Assessments and IAP's for clients on your caseload
  • Actively take part in supervision, staff meetings, in-services and trainings
  • Transport consumers to work sites, meetings, daily activities, appointments, and activities as needed.

Requirements

  • 1 year of experience with the population
  • 1 year of knowledge of medication administration
  • 1 year of knowledge of psychiatric medications, psychiatric and medical treatment
  • Associate Degree in Nursing
  • Valid RN License in Massachusetts
