About the position
The Neuroscience Nurse Navigator Registered Nurse (RN) at Johnston-Willis Hospital plays a crucial role in guiding patients through the care delivery system, particularly for those with neurological conditions. This position involves education, coordination of care, and advocacy for patients, ensuring they receive comprehensive and continuous support from screening to post-surgery. The nurse navigator collaborates with a multidisciplinary team to facilitate appointments, communicate treatment options, and track patient outcomes, all while providing compassionate care.
Responsibilities
- Act as a consultant, facilitator, and educator for the neuroscience population.
- Assist patients through the care delivery system from screening to surgery and beyond.
- Educate patients on their diagnosis and treatment options.
- Coordinate all aspects of patient care and serve as a central point of access.
- Perform initial screening and triage via phone and make appropriate referrals.
- Document patient teaching and coordinate treatment and scheduling.
- Facilitate appointments and consults within established service standards.
- Communicate with referring physicians and interdisciplinary teams about patient needs.
- Facilitate long-term follow-up and track patient outcomes.
- Provide necessary clinical information for pre-authorization and validate its completion.
Requirements
- Current RN License in the Commonwealth of Virginia or compact license.
- Bachelor of Science in Nursing (BSN) or Associate Degree in Nursing.
- 3 years of experience in an adult neurosurgical setting.
- Basic knowledge of care for neurological patients.
- Experience in case management preferred.
- Proficient in the use of computer software.
- Excellent organization, written and verbal communication, and interpersonal skills.
