About the position
The MSW Social Worker at Corewell Health is responsible for coordinating care management and behavioral health interventions for patients with complex care needs. This role involves assessing, planning, and providing therapeutic interventions while collaborating with patients, families, and healthcare providers to promote optimal health outcomes. The social worker will respond to crisis situations and engage with patients to facilitate understanding of their diagnoses and necessary lifestyle changes.
Responsibilities
- Utilizes clinical judgment to coordinate care management and behavioral health interventions.
- Develops strategies for cost-effective, quality care outcomes for patients with complex needs.
- Assesses, plans, monitors, and provides therapeutic interventions as appropriate.
- Collaborates with patients and families to facilitate understanding of diagnoses and necessary lifestyle changes.
- Partners with community agencies to coordinate care across the continuum.
- Acts as a patient advocate to ensure their desires are recognized and followed.
- Participates in care conferences and assists with patient evaluation and treatment.
Requirements
- Master's Degree in Social Work required.
- LIC-Master Social Worker (MSW-Master) or LIC-Master Soc Work LTD in the State of Michigan required.
- Valid Driver's License in the State of Michigan required.
- Basic Life Support (BLS) certification required.
