About The Position
Concordia Visiting Nurses is a Medicare-certified, state licensed, Medicaid approved home health agency operating in nine Western Pennsylvania counties. For well over a decade, Concordia Visiting Nurses has provided high-quality skilled nursing, palliative care, and a variety of therapy services to our patients. Requirements
- A compassionate and caring nurse who is passionate about supporting mothers, newborns, and families during life's most important transitions
- Someone who understands that every family is unique and can provide individualized, family-centered care
- A skilled educator who enjoys helping new and expectant parents build confidence in caring for themselves and their baby
- A strong advocate for maternal and infant health, safety, and well-being
- An independent clinician who is comfortable working autonomously in a home health setting while collaborating with the interdisciplinary team
Responsibilities
- Provide skilled nursing care to expectant mothers, postpartum mothers, newborns, and high-risk infants in the home setting
- Perform comprehensive assessments and develop individualized care plans
- Educate families on prenatal care, newborn care, feeding, safety, and overall wellness
- Monitor maternal and infant health and communicate changes to the care team as needed
- Collaborate with physicians and interdisciplinary team members to ensure quality patient outcomes
- Document patient care accurately and timely
- Support and empower families during pregnancy and the postpartum period
